BRIDGEPOINT APPLICATION PROCEDURE & INFORMATION FOR SASKATCHEWAN RESIDENT ADULTS
FIRST THINGS FIRST ...
Please read all information carefully. Our application package is in three parts. You (client) complete Part A . Part A includes the Referral Data Base, also Questions 1-4.
Part B refers to Service Provider Data, and is usually completed by the Primary Worker. The Primary Worker is your counselor or therapist. He/She is the individual who will be your contact person before, during, and after the program—the person who will commit to being involved with your follow up services. For this reason, if you are not presently working with a counselor or therapist, please approach your local agencies to set up services for yourself or contact BridgePoint for assistance. Once you complete Part A, take the application form to your Primary Worker. He/She will complete Part B, and make a formal referral to BridgePoint by sending both Parts A & B to us. Parts A and B of the application must reach usas soonas possible and at leasttwo weeks prior to the program.These processes take time; therefore we encourage you to begin as soon as possible.
We also require confirmation that you are medically stable. This information comes from your physician. Part C Confidential Medical Information form is to be given to your physician. Your physician will complete this form. He/She will send Part C to BridgePoint two weeks prior to you attending a program. If you are not currently seeing a physician, please contact one. Once again, it is important to consider the timing that is involved to schedule medical appointments, to undertake lab work, and to process the results. It may take up to two weeks to process some lab results, therefore please make your scheduled appointment for one month prior to the start date of the program.
SERVICE PROVIDER REFERRALS (Primary Worker and Physician)
Primary Worker Counselor/Therapist
Ideally clients will have met enough times with their Primary Worker to establish a working relationship. The client's Primary Worker is responsible for *the referral process, *consulting with the BridgePoint Team, as required, during programming, *ongoing client support/counseling, and *follow-up services. He/She will also provide a letter outlining the length of therapeutic relationship, current focus, and any significant clinical information that is necessary to best support the client during her stay at BridgePoint. Refer to Part B for details about the provision of supplemental information.
The Primary Worker will also verify that the client has appropriate information for their own, as well as Family & Friends, participation in programming.

In the event of early program conclusion, regardless of the nature and source of request, a three-way conference call (client, primary worker, and BridgePoint team member) is required prior to the client's departure.
BridgePoint must receive the referral information as soon as possible and at least two weeks prior to the program. BridgePoint will contact the Primary Worker to arrange midway and closure conference calls. Conference calls take place during Modules 1 & 2, providing you (our client) with an opportunity to dialogue about your BridgePoint experience with a member of your Home Support Team.
Physician
Ideally the client will have seen their physician enough times so that a working relationship has been established.
The client's Physician must pre-approve the client's medical fitness for attendance in a BridgePoint program. They must also conduct a medical examination, and complete Part C Confidential Medical Information form for submission to BridgePoint. The physician will also provide any other significant medical information that might be necessary to sustain the client's safety while attending the program, and he/she consult with BridgePoint, as required, during programming. BridgePoint must receive the medical documentation two weeks prior to the program.
STRUCTURE FOR SAFETY
The BridgePoint model is holistic in approach and complements other services. Our team includes a Registered Nurse and we have Medical Health Facilities on call and accessible within 20 minutes (hospitals, doctors, nurses).
BRIDGEPOINT’S CLIENT ASSESSMENT PROCESS
BridgePoint will support the application process, tracking incoming documentation and seeking clarification to any questions that arise. Following our verification that all the necessary components of the referral/application have been satisfied, all forms completed and received, BridgePoint will contact the client to confirm they are registered for participation in the week long men’s intensive program.
FAMILY & FRIENDS PROGRAMMING
Clients who attend our Men’s Intensive Programs are strongly encouraged to choose and invite caring person(s) to participate in specified Family/Friends programming. At BridgePoint, we believe this is an integral part of any recovery process. Please give the enclosed Family & Friends package to people you would like to invite.
Family/Friends programming provides an opportunity to *learn more about eating disorders, *experience some of BridgePoint’s programming, *meet others who are in a similarly supportive role or context, and *explore “being there” for a loved one in recovery.
Please note Family/Friends are responsible for their own accommodation(s). Please see list of available accommodations. Meals are provided at BridgePoint as part of programming, and anyone wishing to make a contribution or donation is encouraged to approach a team member.
