BRIDGEPOINT APPLICATION PROCEDURE & INFORMATION FOR SASKATCHEWAN RESIDENT ADULTS

     Ideally clients will have met enough times with their Primary Worker to establish a working relationship. The client's Primary Worker is responsible for *the referral process, *consulting with the BridgePoint Team, as required, during programming, *ongoing client support/counseling, and *follow-up services. He/She will also provide a letter outlining the length of therapeutic relationship, current focus, and any significant clinical information that is necessary to best support the client during her stay at BridgePoint. Refer to Part B for details about the provision of supplemental information.

The client's Physician must pre-approve the client's medical fitness for attendance in a BridgePoint program. They must also conduct a medical examination, and complete Part C Confidential Medical Information form for submission to BridgePoint. The physician will also provide any other significant medical information that might be necessary to sustain the client's safety while attending the program, and he/she consult with BridgePoint, as required, during programming. BridgePoint must receive the medical documentation two weeks prior to the program.
